Tennessee’s Advance Care Plan An Advance Care Plan is a document that tells your doctor how you want to be treated if you are terminally ill or permanently unconscious. Also known as a living will, Tennessee’s Advance Care Plan was updated in 2004 and includes an area to designate a health care agent. The Health care agent is the person you would want to speak for you in the case that you can no longer make decisions for yourself. You can access Tennessee’s Advance Care Plan and find more resources about advance care planning at thegiftinititative.org [link]
